A personal health record (PHR) is an electronic tool that patients can use to store and manage their health and medical information. A PHR includes information about health and medication history, your family’s medical history and other pertinent information, such as where you live, emergency contact information, allergies, etc. Your PHR can only be viewed with appropriate consent and meant to be available whenever and wherever you and your authorized users allow. The PHR is kept secure with a personal login name and password.
